Our Client is looking for a Patient Advocate to join their team servicing Chicago and North Chicago area ASAP!

This is a part-time, hourly role $60/hr; 12-15 hours/week.

*Must be available during the hours of 9A-3P on working days*

Core Responsibilities

• Clinical Advocacy & Care Coordination
• Coordinate care across healthcare systems to ensure continuity and quality.
• Maintain accurate medication lists and client medical profiles.
• Conduct clinical assessments, medication reviews, and home safety evaluations.
• Support hospital discharges and transitions of care.
• Identify and resolve barriers to care.
  
  

Client & Family Communication

• Explain provider recommendations clearly and understandably.
• Attend medical appointments and translate medical information.
• Reinforce care plans and promote informed decision-making.
• Support client autonomy while respecting personal values.
• Collaboration & Documentation
• Liaise with providers, facilities, and community resources.
• Document all activities accurately and timely in electronic record.
• Maintain accurate billing and reporting.
• Ensure HIPAA compliance and strict confidentiality.
• Professional Development
• Maintain active Illinois RN license.
• Maintain or pursue Board Certification in Patient Advocacy.
• Participate in ongoing continuing education.
  
  

Qualifications/Skills

• Active Illinois Registered Nurse (RN) license.
• Minimum 3 years experience in geriatrics, care coordination, case management, community nursing, OR chronic disease management.
• Psychiatric experience a plus.
• Strong independent clinical judgment.
• Excellent written and verbal communication skills.
• Reliable transportation for local client visits.
• Must have own transport
